When registering a student you will need to fill out the following forms:
You will also need to bring in a copy of your child's
birth certificate and immunization records to register.
Please use this link to find out which school your street address is assigned to.
To request that your child attend a different school, complete the Application for Attendance at an Out-of-District School and return it to the Superintendent of Schools, Dingley Building, 36 Oak St., Lewiston, Maine 04240.
The following forms sent home every school year to be filled out on the first day of school. If your student starts after the first day of school they will be sent home at that time.
